Hong Kong IOSH Community - Picture Accident Analysis

Picture accident analysis is a powerful analytical technique originally developed and used by the U.S. Department of Energy to identify the causal factors behind accidents.

Accidents rarely result from a single failure. They are usually the outcome of multiple conditions, decisions and events unfolding over time. Traditional written reports can make these sequences difficult to follow. Picture accident analysis addresses this challenge by visually mapping what happened and why.

At the heart of this approach are Events and Causal Factors Charts, structured diagrams that clearly display the timeline of events alongside the conditions that influenced them. These charts are widely featured in U.S. accident investigation reports because they make complex incidents easier to understand, communicate and learn from.

During this event, you will:

• Understand the principles behind Events and Causal Factors Charting
• Learn how to read and interpret accident analysis diagrams with confidence
• Explore how multiple sequences of events interact and escalate into incidents
• Discover how to apply this technique in your own workplace investigations
• Strengthen your ability to communicate findings clearly to stakeholders

This is an in‑person event intended for those based in Hong Kong. Please only register if you are able to travel to the venue.
